Paperwork required for Healthcare Marketplace

There is paperwork for people applying for coverage on the Healthcare Marketplace, which opens for enrollment on Oct. 1. Information submitted on the official website, www.healthcare.gov, must include:

• Your Social Security number (or document numbers for legal immigrants)

• Income and employer information found on pay stubs or W-2 forms, for every member of the family needing coverage

• Policy numbers for health insurance plans currently covering you or any family member

• An employer coverage tool form available on www.healthcare.gov/subscribe. It needs to be filled out if you or any family member is eligible for job-based insurance but applying instead to go on the exchange.
